Pharmacological properties

Pharmacodynamics. the asparkam has antiarrhytmic properties, fills shortage of ions of magnesium and potassium. magnesium activates a sodium-potassium atfaza therefore the intracellular content of sodium decreases and hit of potassium in a cell increases. at decrease in concentration of sodium in a cell the sodium-calcium exchange in unstriated muscles of vessels which conducts to their relaxation is slowed down, potassium ions stimulate synthesis of ATP, a glycogen, proteins, acetylcholine; potassium and magnesium support polarization of cellular membranes. asparaginate promotes receipt of ions in a cell and participates in energy balance. the antiarrhytmic effect is implemented thanks to ability of medicament to eliminate an imbalance of electrolytes, to reduce excitability and conductivity of a myocardium.

Pharmacokinetics. It was not investigated.

Indication

For additional therapy in chronic heart diseases (in heart failure, during the postinfarction period, at disturbances of a warm rhythm, first of all in ventricular arrhythmias); additional therapy at treatment by foxglove drugs.

for in/in introductions. the adult to enter slowly in/in by drop infusion — 10–20 ml (to part contents of an ampoule in 50–100 ml 5% of solution of a dextrose). if necessary a dose it is possible to administer repeatedly in 4–6 h the medicament it is suitable for combination therapy.

Course of treatment is defined by the doctor.

Children. Experience of use of medicament for children is insufficient therefore it is not applied at this category of patients.

Contraindication

Hypersensitivity to medicament components; sharp and chronic kidney disease; Addison's disease; av-blockade ііі degrees; cardiogenic shock (hell of 90 mm Hg.).

Side effects

At fast in in administration of medicament should be considered a possibility of emergence of symptoms of a hyperpotassemia, for example nausea, vomiting, diarrhea, paresthesias, and/or gipermagniyemiya, for example face reddening, hyporeflexia, spasms, feeling of heat, respiratory depression.

Special instructions

At long use of medicament needs to control the level of potassium and magnesium in blood and also regular monitoring of indicators of an electrolytic hemostasis and data of the ECG is necessary.

Asparkam as the medicament containing potassium and magnesium needs to be applied with care at patients with a myasthenia gravis; at states which can lead to a hyperpotassemia, such as acute dehydration, widespread damage of fabrics, in particular in heavy burns. At this category of patients it is regularly recommended to investigate the level of electrolytes in blood serum.

As medicament contains sorbite, patients with hereditary intolerance of fructose should not use this drug.

Use during pregnancy and feeding by a breast. Still it was not reported about emergence of any danger at use of medicament for this category of patients.

Interaction

Simultaneous use of medicament with kaliysberegayushchy diuretics and/or inhibitors apf increases risk of development of a hyperpotassemia.

Cases of overdose are unknown to

. in case of overdose it is possible to observe symptoms of a hyperpotassemia and a gipermagniyemiya.

hyperpotassemia Symptoms: general weakness, paresthesias, bradycardia, paralysis. Extremely high concentration of potassium in blood plasma can lead to a lethal outcome from oppression of warm activity, arrhythmia or cardiac arrest.

gipermagniyemiya Symptoms: nausea, vomiting, drowsiness, hypotension, bradycardia, weakness, the muffled speech, doubling in eyes. At very high plasma concentrations of magnesium the hyporeflexia, paralysis of muscles, an apnoea and cardiac arrest can develop.

in case of overdose needs to cancel K ⁺ - Mg ²⁺ - aspartate and is recommended to carry out symptomatic treatment (Calcii chloridum of 100 mg/min. in/in, dialysis if it is necessary).
